Job Summary The primary role of the Medical Social Worker II in the Hospice/Home Health setting is to assist patients and families/caregivers in coping with social/emotional issues and practical arrangements related to the patient's illness. Under general direction, provides age-appropriate social work care to patients and their caregivers in their residence, following agency policies and regulations. Serves as an integral part of the home health team, providing assessments, coordination, and implementation of social work services to meet complex patient needs. Essential Responsibilities Provide psychodynamic interventions, crisis intervention, grief/bereavement counseling, problem solving, stress reduction, and developing healthy coping strategies in individual, family, and group settings. Offer counseling to help patients cope with acute, chronic, or terminal illnesses. Develop and implement individual plans of treatment to help patients and families cope with or restore social, emotional, financial, and environmental factors affected by illness. Complete biopsychosocial strengths-based assessments. Partner with patients to identify needs and develop mutually agreed-upon treatment plans. Discuss care options proactively, including Kaiser resources and external community or government resources, to aid in developing short- and long-term care plans. Collaborate with other disciplines to assess, plan, and provide services using biopsychosocial information. Assist patients in advocating for themselves to receive appropriate services within Kaiser and the community. Support patient and family in placement in higher levels of care as needed. Review, evaluate, and prioritize referrals. Maintain documentation, records, and data collection accurately and timely. Participate in ensuring appropriate, timely placement of Kaiser members in nursing facilities. Serve as a liaison between patients and Kaiser, maintaining positive relationships and continuity of care. Identify appropriate care levels and facilities for referred patients and obtain placements when applicable. Coordinate with internal and external resources to meet goals and objectives. Provide information and referrals to community resources. Manage the exchange of information between Kaiser, families, members, and skilled nursing facilities, including benefits application. Participate in Quality Assurance activities. Coordinate communication among regional offices, clinics, hospitals, and field staff, including triaging calls from members/families. Clarify and complete required clinical and psychosocial information with referral sources. Perform other related duties as necessary. Experience & Education Masters in Social Work accredited by the Council of Social Work Education. Step I: 0-2 years social work experience. Step II: 2-4 years within the last five years. Step III: 4+ years within the last ten years. Licenses & Certifications Driver's License (California) Basic Life Support certification Additional & Preferred Qualifications Ability to work effectively on a multidisciplinary team. Strong psychosocial assessment skills. Knowledge of disease impacts on patient and family functioning. Excellent oral and written communication skills. Computer literacy and experience with clinical information systems preferred. Ability to handle demanding workload efficiently. Independence and teamwork capability. Willingness to work in a Labor Management Partnership environment. LCSW preferred; MSW internship experience considered.
